MR Guided Focussed Ultrasound - MRgFUS

MRgFUS refers to magnetic resonance guided focused ultra sound. It is a non invasive technique used in the treatment of symptomatic uterine fibroids and benign or malignant tumors. This technique involves the use of focused ultrasound energy to heat the fibroids until the tumor is ablated. This is done by focusing on a small target area and raising the temperature high enough to destroy the target without damaging the surrounding tissue. The treatment is guided and controlled with the use of magnetic resonance imaging (MRI). The patient lies in the MRI scanner and the tumor is located with the help of the images provided by the machine. 

Symptoms

MRgFUS can be used in the treatment of the following conditions:

  • Bone metastasis- refers to the spread of cancer to the bone. It is often characterized by pain with tumors in the ribs, shoulders, pelvis, sacrum etc. to use MRgFUS as treatment, the tumors must be visible on an MRI and accessible to the ultrasound beam
  • Fibroids- are benign tumors in the uterus and can present with symptoms like intense pain, heavy menstrual bleeding, abnormal bleeding, pain during sex etc.
  • Osteoid osteoma- benign bone tumors in children and adults, mostly originating in the long bones like the femur (thighbone)
  • Essential tremor- a chronic neurological condition that causes involuntary tremors in parts of the body like the hands and arms and sometimes the head or legs

Diagnosis

The need for an MRgFUS can be confirmed through the following diagnostic conditions:

  • Medical history and physical examination
  • X-rays
  • MRI and CT scans
  • Ultrasound 

Treatment

The skin over the target area is shaved and prepared for treatment. An intravenous catheter will be placed in the arm to administer fluids and medications during the procedure. The patient is positioned such that the body is in direct contact with the transducer gel pad. Heart and lung function is monitored closely during the procedure. Before the procedure begins, the patient will be given a stop sonication button. If there is any problem or discomfort during the procedure, the patient can press this button to stop the treatment. The patient will then be moved inside the MR device and the entire procedure would be controlled and supervised from an adjacent room. A number of images are then taken in order to plan the treatment. The area to be treated is marked, taking precautions to avoid vulnerable areas. The procedure begins with low sonications, gradually increasing in intensity to reach full strength. Each sonication is about 20-30 seconds long with a cooling period of 40-70 seconds between each sonication. Once the treatment is over, MR images will be taken to gauge results.

After Procedure

1) What is MR guided focused ultrasound?

(MRgFUS) Magnetic Resonance-guided Focused Ultrasound uses ultrasonic pulses to heat up and kill fibroid tumors which are abnormal, usually harmless growths in a woman's uterus. MRgFUS is also known as focused ultrasound surgery. The procedure uses MR guidance to accurately target the fibroids and spare healthy tissue.

2) Do patients experience pain?

The most frequent side effects of MRgFUS treatment, as stated by patients, include slight discomfort and pain. The discomfort caused by lying on their stomach on the scanner for a prolonged period is one of the most frequently reported complaints among patients. The MRgFUS beam can also cause pain during the treatment if therapy cells are positioned close to the fibroid capsule.

3) How is MRgFus performed?

MRgFUS is performed on an outpatient basis. The patients return to normal activities within a few days. During the procedure, the patient is asked to lie on her stomach in an MRI scanner. The procedure is usually performed for three hours. The woman is kept under conscious sedation while the high-intensity ultrasound waves from the ultrasound probe are focused on small regions in the fibroid. This causes the temperature of the fibroid to rise, resulting in cellular death of the fibroid cells. After approximately 1 to 2 hours of recovery, the patient is able to leave the hospital.

4) How effective is MRgFUS?

MRgFUS is highly effective, with most patients reporting some immediate improvement in their symptoms within a month of the procedure.

 

5) What are the known (most common) complications associated with the treatment?

MRgFUS is generally a safe method, with the most common side effect of the treatment including discomfort and minor pain, such as fatigue and back pain following treatment.  However rare, the most severe complications include persistent buttock and leg pain (Connected to heating of the sciatic nerve) as well as skin burns.

6) What type of anesthesia is used during MRgFUS?

The procedure is performed under local anesthesia while the patient is also mildly sedated.

7) What is the rate of recurrence after MRgFUS?

The results have shown that the rate of recurrence of fibroids is lower than and sometimes comparable to the rate with other uterus-preserving therapies. The reported rate of recurrence was 7.4% after one year, 14% for type 1/2 fibroids and 21.7% for type 3 fibroids after two years. 

8) What is the fibroid shrinkage pattern after the therapy?

The fibroid continues to shrink for 1 year following ablation. But the majority of the shrinkage occurs in the first 6 months. 

9) Does the dead tissue that is left inside the body after the treatment cause an infection?

No, the dead tissue stays intact in the fibroid sac and therefore does not come into contact with the surrounding tissue and does not causes infection.

10) Is MRgFUS covered by insurance?

MRgFUS is not covered by many insurance policies.
